הכתובה מיום רביעי כ' בכסלו שנת 5664 לבריאת עולם, "כאן במתא צפרו יע"א".
החתן: מאיר בן חיים יוסף דוד בן אליהו בן שמעון רפאל דוד בן ישועה אזולאי
כלה: חנה בת אלישע בן שמואל בן משה אג'ייני
עדים: אבא אלבאז, ומכלוף אליהו אדהאן.
מתחת לכתובה תנאים נוספים, ביניהם התחייבות החתן "שלא יעכב על הכלה ... בשום זמן מללכת אל בית אביה וקרוביה ... [ו]שיהיה מגלגל עם הכלה הנז' בענין תשמיש מיום כניסתה עמו לחופה ועד שתגיע לפרקה".
עם הכתובה צולם גם מסמך ביטול הכתובה הראשונה, וכתיבת תנאי כתובה חדשים. נעשה בי"ב באדר ראשון תש"ו, בחתימת יקותיאל מיכאל אלבאז ויהושע מאמאן.
