פנקס מודפס (גרמנית) וממולא בכתב-יד (הונגרית ומעט עברית). הפנקס מכיל רישומים של למעלה מ-1300 לידות ובריתות באיזור הערים פוטנוק בהונגריה וטורנלה בסלובקיה. כולל רישומים מעשרות מקומות יישוב. כל רישום כולל את הפרטים הבאים: מספר סידורי, תאריך הלידה, שם ומין הילוד, שמות האב והאם (כולל שם משפחת האם קודם לנישואיה), מקום המגורים ומקום הלידה, שם המיילדת, לבנים: יום המילה ושמותיהם של המוהל והסנדק, לבנות: יום נתינת השם. כמו כן צוין אם הילד נולד מחוץ לנישואין. בסוף כתב היד חתימתו וחותמתו של יעקב טננבאום מפוטנוק.
