    פותח: "אמר המלקט ...אני יהודה בן יצחק בן אברהם בן שושאן הספרדי"

    מתוך תיאור כה"י ב"אוצר כתבי-יד עבריים מימי-הביניים": "תחילה החזיק הקונטרס 15 דפים, והחיבור הסתיים בדף 15א [היום: 17א]. לימים עקר המחבר ממקומו ל"פרובינציא", ושם צירף תוספת לחיבורו על יין-נסך. תוספת זו נכתבה בדיו בצבע שונה על גבי הדף האחרון של הקונטרס המקורי ועל-גבי שלשה דפים נוספים [20-18], שבהם טבוע סימן-מים השונה מסימן-המים הטבוע בגליונות הקונטרס המקורי". התוספת פותחת: אמר יהודה הנזכר אחר שעוני טלטלני ... עד כי לארץ פרובינציה הביאני ראיתי כי רוב המון יהודים אשר בה מזלזלים באיסורי המאכלות" ומסיים: "הנגש ונענה ... בהר חר"ב ... יהודה מבני שושאן מכונה"

    בדף 2א: מכתב אוטוגרפי מאת המחבר "יהודה בן שושאן", כנראה למאן דהוא אליו שלח את ספרו. בסופו (20א): שאלה ששלח המחבר בכתב-ידו ותשובה קצרה בכתיבה אחרת
