نسخة كتبت سنة 967 هجري / 1559 على يد نور الدين محمد بن تاج الدين حسين الحسيني في مدينة قزوين.
أول النسخة: "الحمد لله الذى توحد بابداع الآحاد وتفرد بتأليف صنوف الاعداد الخ"؛ صفحة 91ب: "صورة خط المص حرره مؤلفه .. جمشيد بن مسعود الطبيب .. \في 3 جمادى الأول 830 هجري \ (1427) و26 تيرماه سنة زصو يزدجردية ثم نقلت هذا الكتاب لنفسى .. من خط المنقول من خط المصنف .. ووقع الفراغ الخ".
