"ניצחון הרפואה בעידן המודרני מפליא בהישגיו. היא התגברה על סכנות מאיימות והדבירה מחלות רבות. עם זאת, הרפואה – על כל חידושיה ותגליותיה – לא השכילה להתמודד עם סוגיית ההזדקנות ועם סוגיית המוות. המוסדות שהקימה החברה לטיפול בזקניה דואגים לרווחתם של המטופלים, למזונם ולביטחונם האישי, אך נמנעים ממפגש עם המצוקה האמִתית של הנתונים לפיקוחם: המוות הקרב ובא וכיצד להתמודד עמו. במקום לשפר את ימיו של הזקן הקרב אל קִצו, הם משתדלים להאריך אותם. גוואנדה ביקר לצורך מחקריו המסועפים, שאת ממצאיהם הוא מביא בספרו, בעשרות מוסדות סיעוד ונפגש עם עשרות מזדקנים ונוטים למות. הוא לא היסס לשלב בכתיבתו את ניסיונו מהטיפול הכאוב בבני משפחתו הקרובים. גוואנדה מספר לקוראיו גם על נקודות האור האופטימיות במצב קשה זה: אחיות, רופאים ומטפלים שמצאו נתיבים אל ליבות הנוטים למות ומקלים עליהם את שעותיהם הקשות" -- מן המעטפת האחורית.
